Here is a quick look at the procedure to get a commercial septic permit in Indiana.

1  Hire a Professional Soil Scientist to evaluate the proposed septic site
   http://www.isco.purdue.edu/irss/roster.html
   
2  The soil's scientist will send their report along with information regarding your facility to the Indiana State
    Department of Health for evaluation.

3  The State Department of Health will then issue septic requirements based on the soil report and estimated
    daily flow of your project.  This septic requirements report from the State is called a Data Sheet.

4  The data sheet is then sent to the client and to the Health Department

5  A septic design is then produced (and stamped by a Professional Engineer or Architect) and sent to the State for review along with a completed application and fee (usually $200).

6  An approval letter is issued once the State has reviewed and approved the plans (revisions may be necessary).

7  The approval letter will be sent to the client and copied to the local County Health Department

8  Follow your local County Health Department requirement's to get your permit.

Note:  Low flow systems (=<750 Gallons per day) may be forwarded to your local county Health Department for plan review.
